Static checks passing does not mean the fix works; the server never runs submitted commands.

Context

Source(s): https://community.openai.com/t/codex-extension-issue-in-vs-code/1379758 Signal: 22,189 views, 28 posts. Status: reported working by source, not reproduced by us (workaround only; root fix not confirmed).

Problem

After the April 2026 extension update the Codex panel never finishes loading, even after reinstalling. Seen on Windows and macOS, also in Cursor.

Error (redacted / normalized)

Unable to create new task timeout

Not this when / boundary

Remote SSH to a Windows host without WSL fails with spawnSync wsl.exe ENOENT; rolling back didn't fix that. Not the same as fix/58 (Remote WSL webview HTTP 401).

Root cause

Not confirmed. One user blamed corrupted local Codex state.

Fix

  1. Roll back: Extensions > Codex > gear (or the dropdown next to Uninstall) > Install Specific Version > 26.415.20818, then reload. One user said only this worked for them.
  2. Or reset local state: rename ~/.codex to a backup, restart VS Code, sign in again, then copy your old sessions back from the backup. A user reported this or the newer 26.422.62136 release fixed it.
  3. Update to 26.422.62136 or newer (one user wasn't sure whether this or the reset fixed it; another said pre-release 26.5422.71525 fixed it).

How to verify

The panel loads and a new task can be created.
